| Description | As found with the documentation: NA 65/1955 SUMMARY: This 22 year old Caucasian woman, married and with 4 children, was admited to the Research Hospital on Feb. 28th, with a history of losing consciousness in her physician's office while climbing onto a table for diathermy treatment. After that she continued to vomit and had a loss of memory for a few days following her loss of consciousness. On her admission, she seemed to be in an improved state. However, on March, 15th, while talking to a nurse, she suddenly lost consciousness and remained comatose until her death which occurred on March 19, 1955. A right carotid arteriogram study, performed on March 14th, revealed filling of the right internal carotid with a large aneurysm at the bifurcation of the anterior and middle cerebral arteries. The diagnosis of an aneurysm of the right internal carotid artery was made. The day before death, the patient had gross hematuria. The autopsy revealed an aneurysm of the right internal carotid artery at the site of the bifurcation as expected. There was old and recent subarachnoid hemorrhage, and intraventricular hemorrhage. The body viscera were normal except for the right kidney where a triangular, yellow, 3cm. in diameter, well circumscribed area was present similar to an infarct. However, no occlusion was found in the renal vessels. The immediate cause of death was the subarachnoid and intraventricular hemorrhage, secondary to rupture of the saccular aneurysm of the right internal carotid artery. On gross inspection, the brain appears to be normal except that there is obvious softening of the right frontal lobe. There is some evidence of blood in the subarachnoid space in the basal cisterns extending on the underneath surface of the cerebellum and there is blood in the fourth ventricle. When the circle of Willis has ben cleaned, it is seen that there is a saccular aneurysm arising from the region of the bifurcation of the right carotid artery into its two terminal divisions. The aneurysm itself appears to be 7x5 mm. and is not obviously connected with the large clot which is to be found in the right frontal lobe. The clot has obviously extended into the ventricular system on both sides. The surrounding brain is swollen and soft. PROVISIONAL DIAGNOSIS 1. Intraventricular hemorrhage, severe, with encephalomalacia. 2. Old and recent massive subarachnoid hemorrhage. 3. Ruptured saccular aneurysm of the right internal carotid artery (congenital?). 4. Infarct of the right kidney (?). |
